Birth Registration

Registration Period

Within 14 days from the day the child was born
 
※The day of birth will be counted as the first day.
※If the 14th day is a holiday, can be extended until the following working day.

Person Eligible to Register

  • The father or mother of the newborn child
  • If another person will submit the birth registration form, the seal or signatures of the mother and father of the newborn child will be required.

Place of Registration

  • Domicile of parents
  • Address or place of residence of the person who registered
  • Place of birth or the municipality location of the hospital where the baby was born
  • In Ichikawa City Hall, registration is accepted at the Minami Gyotoku Civic Centre, Gyotoku Branch Office-Citizens Affairs Section, Ogashiwa Branch Office, and at the Ichikawa City Hall Main Office-Citizens Affairs Section

Necessary Documents:

1.Birth Registration Form

Birth Registration form, where at the right side of the birth registration form, is the space for the names and signatures of the doctor or the midwife.

2.Personal Seal of the Person Eligible to Register

Stamp-type of seal will not be accepted.

3.Health Pocket Book of mother and child
